IS THERE ANYTHING I NEED TO DO?
YES, PLEASE. Please follow the below instructions in order to ensure your records are transferred to the new office. Federal law requires that patients or their parents/guardians give written authorization before records are transferred from one medical entity to another. 


This is especially important for for those families who already have appointments scheduled with Dr. Davari for the months of February, March or April, please follow the below instructions to ensure that your appointments are activated at the new office for the same date and time as currently scheduled.

    INSTRUCTIONS for MEDICAL RECORDS REQUESTS

  1. Please note, Records requests made after our last day of business (March 27, 2020) will require a $50 fee per patient for retrieval of medical records from storage. Please allow 14 business days once payment and release form are received.
  2. Please complete and sign the Medical Release Form that you received in the mail or download the form by clicking HERE. Please make sure to write VERY LEGIBLY and complete the form in its entirety.
  3. Please mail the completed form along with a check payable to "Vivian Liu, MD, A Medical Corporation" for the appropriate amount to the following address: 425 15th St #3471, Manhattan Beach, CA 90266 (Please include the "#" symbol for proper delivery).
  4. Please contact your insurance carrier to ensure that UCLA is an accepted provider group. To see a list of health plans accepted by UCLA, please visit www.uclahealth.org/health-plans.


WHEN WILL YOU BE MOVING?
Dr. Davari moved on February 1, 2020. Dr. Liu will move April 2020. Our office is closed as of March 27, 2020.

The suite is in the brand new modern office building on Sepulveda Blvd just south of Manhattan Beach Blvd. (South of the Manhattan Beach Target store and kitty-corner to Baskin Robbins)
Note: This is NOT the old UCLA pediatrics office on Sepulveda and Marine. Nor is it the UCLA Radiology office on Sepulveda and 22nd.

WHAT INSURANCES DOES UCLA ACCEPT?
To see a list of health plans accepted by UCLA, please visit www.uclahealth.org/health-plans. While we do not expect any problems with insurance during this transition, we ask that you please contact your insurance carrier to ensure that UCLA is an accepted provider group for your plan. 

CAN I HAVE MY RECORDS TRANSFERRED TO ANOTHER PRACTICE?
While we would hate to lose you, we completely understand that you may need or wish to be seen elsewhere. In order to have your records transferred anywhere other than UCLA, please check the blank box on the right of the Medical Release Form and fill out the name, address, phone and fax number of the receiving person or office.

Absolutely! We’re honored to take care of your families and hope to continue to do so!

IS THERE PARKING AT THE NEW OFFICE?
Yes, there is free parking underneath the building as well as a small parking lot off 10th street immediately outside of the UCLA Manhattan Beach Pediatrics office.

HOW DOES SCHEDULING WORK FROM NOW ON?
UCLA offers same-day appointments for well-child physicals as well as for sick visits. Appointments can be made by phone or online. Their office number is as follows: (310) 546-8702.

CAN WE CONTINUE TO SEND QUESTIONS BY EMAIL?
You can communicate with us via the UCLA Portal messaging system. Dr. Davari and Dr. Liu’s email addresses will be retired in February and April of this year, respectively.
