OUR ROSECRANS OFFICE

​We are excited to announce that we have now joined UCLA Health and relocated to their brand new office space located at 1000 N. Sepulveda Blvd in Manhattan Beach - one mile south of our old Rosecrans office (just south of the Manhattan Beach Target, kitty corner to Baskin Robbins). Note: this is a brand new building, not the older UCLA-branded building on Sepulveda and Marine.


Please follow the below instructions in order to ensure your records are transferred to the new office. Federal law requires that patients or their parents/guardians (in the case of minors) give written authorization before records are transferred from one medical entity to another. 

    INSTRUCTIONS for MEDICAL RECORDS REQUESTS

  1. Please note, Records requests made after our last day of business (March 27, 2020) will require a $50 fee per patient for retrieval of medical records from storage. Please allow 14 business days once payment and release form are received. 
  2. Please complete and sign the Medical Release Form that you received in the mail or download the form by clicking HERE. Please make sure to write VERY LEGIBLY and complete the form in its entirety.
  3. Please mail the completed form along with a check payable to "Vivian Liu, MD, A Medical Corporation" for the appropriate amount to the following address: 425 15th St #3471, Manhattan Beach, CA 90266 (Please include the "#" symbol for proper delivery).
  4. Please contact your insurance carrier to ensure that UCLA is an accepted provider group. To see a list of health plans accepted by UCLA, please visit www.uclahealth.org/health-plans.
